Sample collection kits are shipped to your desired location OVERNIGHT FREE OF CHARGE. Healthcare providers select their desired form of payment on the Requisition Form provided with the sample collection kit. Currently, customers may choose from the following payment options: Bill Private Insurance, Bill Medicare, Bill Medicaid, Bill Account (for qualified customers) or Patient Self-pay (Certified Funds, Check, Money Order, Major Credit Card). Please contact one of our Customer Care representatives for pricing information.
